Lawrence leads Aussies into in World Cup Semi Finals
At the opening ceremony of the ICF World Cup event at the Eiskanal in Augsburg the organizers honoured participants from the 1985 World Championships.

Tips & Hints: Wrist Pain from Overuse - 'Intersection Sydrome'

Wrist pain can be caused by overuse as a result of having too tight a grip on the paddle. One of the common overuse injuries is Intersection syndrome, which is tendinitis or tenosynovitis in the first and second dorsal compartments of the wrist.

Holding the paddle with a tight grip for extended periods of time, (e.g. in white water, marathon or sea kayak paddling) paddling puts a high impact on the wrists and forearms and therefore creates a risk for this overuse injury. Twisting the paddle parallel makes further stresses to the forearm as it is a repetitive extension motion at the wrist.

Affected paddlers typically experience pain, tenderness and swelling at the wrist in the region of the dorsal forearm. While examining the affected forearm one will notice point tenderness on the dorsum of the forearm as well as squeaking with passive or active motion and visible swelling along the affected tendons. In some cases there is even a palpable grating with passive or active motion in this area, which can even extend proximally to the mid-forearm. The range of motion remains normal, but the paddler experiences pain on active wrist extension and deviation. As a result of the pain the strength is slightly decreased on almost every motion (extension, deviation, supination) and grip.

These overuse symptoms normally occur early in the outdoor paddling season when feathering the paddle is still an unaccustomed activity. Within elite athletes it also represents an occupational hazard.

Treatment involves rest in the first place and technique modification in the long run.  Paddlers who tend to be affected by this overuse injury should try to keep the paddle with their wrists as flat as possible besides loosening their grip. Medical treatment can consists of wrist splinting, avoidance of exacerbating activities, icing, and corticosteroid injection into the tendon sheath next to rehabilitation exercises. Usually tendinitis is resolved fairly quickly given appropriate management but in resistant cases surgical release of tendon sheath is unavoidable.
